Step 1:
If you would like to join our waitlist to become a patient at the Dorset Health Hub, please complete the following Intake Form (pdf).
We prioritize patients who do not have a family doctor/nurse practitioner. To become a patient of the hub you must live within a one hour radius from our clinic.
Step 2:
Once you have completed the form, please send it to the Hub. You can mail, fax, or drop off the form to the address below:

Step 3:
Once we receive your completed form, we will review and assess your form for eligibility to become a patient of the Dorset Health Hub. If eligible we will then CALL you for an intake appointment when a spot opens up. If you are on any medications, please bring ALL of your medications in their bottles to your first appointment. At this appointment you will meet the nurse practitioner and final decisions regarding becoming a patient will be made.
