Becoming a Patient at the hub

 

Step 1:

If you are currently an unattached patient, or have a family doctor or nurse practitioner elsewhere and are looking to be connected to a local provider please ensure you have followed the instructions on Ontario Health Care Connect, or call 811. This will place you on an Ontario-wide waitlist for primary care providers where they will attempt to attach all Ontarians to local providers as they become available. This is strongly recommended for anyone interested in becoming rostered with The Dorset Health Hub.

Step 2:

To become a patient at the Dorset Health Hub, you will need to complete an intake form, if advised please complete the following Intake Form (pdf), or give the Hub a call (705) 766-0866.

We prioritize patients who do not have a family doctor/nurse practitioner. To become a patient of the hub you must live within a one hour radius from our clinic.


Step 3:

Once you have completed the form, please send it to the Hub. You can fax, mail, or drop off the form to the address below:

Step 4:

Once we receive your completed form, we will review and assess your form for eligibility to become a patient of the Dorset Health Hub.

If eligible we will then CALL you for an intake appointment when a spot opens up. If you are on anymedications, please bring ALL of your medications in their bottles to your first appointment. 

NOTE: This is dependent on Nurse Practitioner availability, meaning we cannot guarantee a timeline.
